HCP is now a (CDO) Counselor Designated Organization with MassHealth and the Health Connector!

HCP has joined Health Care For All in achieving healthcare equity by removing barriers to healthcare access, promoting culturally competent care, improving health literacy, and ensuring that healthcare services are affordable, available, and appropriate for all individuals.  In meeting our mission, HCP has been designated as a CDO.

Certified Application Counselor Designated Organizations (CDOs) are a vital component of the assister community in Massachusetts.  CDOs oversee Certified Application Counselors (CACs) who are trained and able to help consumers seeking health insurance coverage options through the Commonwealth or the Federal Marketplace.  HCP has three Certified Application Counselors (CACs) trained and able to help applicants looking for health coverage options through the MassHealth (Medicaid) program and the Health Connector enrollment process. HCP is an active community-based organization assisting in the MassHealth redetermination program.  HCP services are free!  HCP office hours for all MassHealth recertifications are Wednesday, Thursday and Friday 1:00 PM – 4:00 PM.  You may also request an appointment an appointment by telephoning 508-588-0400.  It is essential to note the Federal Medicaid continuous coverage requirements have been in effect since March 2020 and will expire in April 2023.  ACT NOW! About Haitian Community Partners Foundation

Haitian Community Partners is a Brockton-based organization comprised of Haitian expatriate professionals. Our mission is to work collaboratively with community-based and national organizations to improve access for the underprivileged in the areas of social services, immigration, youth leadership, and health that are appropriate, comprehensive, integrated, and culturally sensitive.  By offering a range of services, educational programs, and advocacy initiatives, we aim to address the unique needs and challenges faced by migrants and those that face economic and social inequalities.

HCP’s Law Clinic is born from systemic problems in the immigration enforcement system, such as racial and economic bias, law enforcement and accountability, hyper-incarceration, and access to justice—our Immigration Law Clinic partners with South Coastal Counties Legal Services, which provides legal advice to migrants. Our staff are graduates of the Immigrant Legal Resource Center’s Basic Immigration Law Course, where they learned legal skills, such as interviewing, client counseling, fact investigation, legal research and writing, and INS form preparation. Legal advice is the exclusive responsibility of licensed attorneys of South Coastal Counties Legal Services, where constituents meet privately with an immigration attorney to discuss the immigration process.  Our clinic’s dates and times are posted on our website, and we do offer mobile clinics at various locations sheltering newly arrived migrants.

HCP’s youth leadership program is structured on five pillars to foster the development of leadership skills and qualities in Brockton’s young adults.  We provide young adults opportunities to enhance their leadership abilities, self-confidence, critical thinking, communication skills, and problem-solving capabilities.  Through educational and experiential activities, workshops, team-building exercises, and community service opportunities, we provide a well-rounded learning experience.  Become a youth organizer today, and you may be president tomorrow.  Join our Youth Leadership team.  Certificate program with a small stipend.  We look forward to hearing from you!

Our ESOL Program serves the community by providing comprehensive, learning-centered, high-quality English language programming that help students reach personal, educational and professional goals, and that are scheduled to accommodate busy work and home lives.  Seating is limited and we currently have a long waiting list.  You may submit your application online or in person during normal business hours.

HCP’s migrant support and resources provide various types of support and services to migrants, refugees, asylum-seekers, and other displaced individuals or families.  Our mission is to assist migrants in transitioning to America and the Commonwealth.  We try to reduce challenges by promoting migrants’ well-being and integration into America.  Our services include legal aid referral and our clinics for seeking asylum or facing immigration-related issues; language services, such as translation and interpretation, to facilitate communication between our migrants and various service providers, including healthcare professionals and government agencies. Education supports getting children to register for school, and ESOL assists in English-speaking skills for work readiness.  We assist migrants in obtaining health insurance as Certified Assistance Councilors in navigating the healthcare system and connecting them with medical services and resources.  We have developed a welcome packet assisting new migrants in community integration.  HCP engages in advocacy and policy support, to protect the rights and well-being of migrants, by participating in policy discussions and initiatives related to immigration and human rights.
